Thank you for choosing our office for all your medical care needs. To assist us with your care, please arrive at least 15 minutes early to complete registration forms if you have not already printed and filled them out. Please bring the following items to your first visit:
- Your insurance card and a photo ID
- A list of medications with dosage that you are currently taking – or your medication bottles
- Results of any related medical or laboratory tests
- Names and phone numbers/addresses of other doctors you are already seeing

Our physician and nurses are experienced in the latest advancements in internal medicine. We believe that good medical care begins with open communication.
